Eclipse Athlete Training Insurance

  • Coverage limit of $5 million per incident

  • $2 million for Directors & Officers (Wrongful Acts/Error & Omissions)

  • Includes coverage for the Directors of Eclipse Athlete Training

  • Protection for all athlete training events and activities organized by Eclipse Athlete Training

  • Additional Insureds: Directors, Coaches, Trainers, Managers, Officials, and Volunteers

  • Coverage for both participants and third-party risks (including spectators)

  • Includes riders such as Cross Liability, Non-Owned Automobile, and Tenants Legal

  • Nationwide coverage across Canada

  • Includes incidental Medical Malpractice coverage

  • Advertisers and Employers Liability coverage

Treatment under (C) & (G) must be completed within 52 weeks from date of accident.

  • Note: There is no Coverage for Loss of Wages

  • Physiotherapy treatment is $500 per year; no limit/visit. Physician referral/prescription is required.

  • If you incur medical or dental expenses as the result of a sports injury, you are required to submit those expenses to Alberta Health Care, Blue Cross or Employer plan, firstly. This is secondary coverage to any other insurance in force (work plan, etc.)

INSURANCE CLAIMS INFORMATION

Eclipse Athlete Training - Insurance Claim Process

To ensure all insurance claims are properly processed, please follow this sequence:

  1. Alberta Health Care: First, contact Alberta Health Care to file your claim.

    • If Alberta Health Care does not fully cover the claim, proceed to the next step.

  2. Blue Cross: If you have a Blue Cross plan, contact them second to file your claim.

    • If Blue Cross does not cover the full amount, move on to the next step.

  3. Parent’s Employer Health Plan: Contact the parent’s employer health plan third to file your claim.

    • If the employer health plan only partially covers the claim or does not cover it at all, proceed to the next step.

  4. Eclipse Athlete Training: Finally, if any balance remains or if the claim is not covered by the previous plans, submit the claim to Eclipse Athlete Training. Our insurance will cover the approved claim or any remaining balance.

  5. Claim Submission: Notify Eclipse Athlete Training of any claims being submitted, including the player’s name and the nature of the injury. Follow the instructions on the claim form and submit all required information to the address listed on the form, as well as a copy to Eclipse Athlete Training.

    • Group Policy Number: Be sure to ask Eclipse Athlete Training for the group policy number before filing your claim.

  6. AMBULANCE Claims: If a player requires an ambulance, provide the parents with the claim form and instructions, in case they don’t have proper coverage.

  7. Team Registration: Insurance coverage is valid only if the player, coach, or other personnel's name is listed on the team registration form, which must be on file in our office.
